Description

To request a replacement passport for an APSL Lusitano:

• This form must be filled out in order to request a replacement passport for your horse. Form found HERE.
• The replacement passport must be requested by the owner of record.
• There may be only one passport in existance for any horse. So if the original was damaged, it must be returned to USLA for return to APSL.
• The price of the replacement passport depends on your level of membership. To check pricing, please click HERE.
• After you submit the form, the USLA Team will request the necessary original document from Portugal. The document will be sent to USLA, and we will forward it to you.
• The passport document must be filled out by your veterinarian and then returned to USLA.
• USLA will return the passport document to APSL, who will complete making the new passport book. When USLA receives the passport document, we will send it to the requestor.
